Zusammenfassung

Facial middle third reconstruction is one of the most difficult challenges in maxillofacial surgery. We review our Department experience in major middle third defects reconstructive surgery between 1990 and1998, (84 cases), we also analyse the aesthetic and functional result obtained. We do not consider minor defects that can be reconstructed with local flaps or primary closure. We reveal our protocol to this defects, and indicate the reconstructive option performed taking into account the defect shape and localization. The pros and cons, and the complications we had are commented. We made a bibliographic search of middle third reconstructive procedures. We think, that primary facial middle third reconstruction have to be done after oncological ablative procedures. Showed protocol obtaines good aesthetic and fuctional results. We chose the best result reconstructive procedures taking into account the procedure morbidity and complexity versus its aesthetic and functional advantages.
