La migraña es la segunda cefalea primaria más frecuente después de la cefalea tensional. En ocasiones produce una alteración significativa de la calidad de vida de quien la padece. Es fundamental el diagnóstico y el tratamiento abortivo de la crisis como el tratamiento profiláctico de las mismas. Para un adecuado manejo habrá que individualizar el tratamiento dependiendo de la edad, del sexo y de otras comorbilidades que sufre el paciente migrañoso. Los tratamientos de las crisis de migraña, así como los profilácticos se deberán aplicar en las dosis necesarias o en las combinaciones sinérgicas, para mejorar la efectividad y evitar los efectos secundarios. Para el manejo se tendrá en cuenta no solo el tratamiento farmacológico, sino también la evitación de los factores desencadenantes o intensificadores. Hay determinados tipos de migraña de aparición menos frecuente, pero que si no se piensa en ellas pueden hacer sufrir al paciente un tratamiento equivocado, innecesario e incluso peligroso, así como la petición de pruebas innecesarias. Siempre ante un paciente migrañoso habrá que realizar una correcta anamnesis y exploración neurológica para detectar los signos o síntomas de alarma que nos hagan realizar una prueba de neuroimagen o la derivación a un neurólogo.
