Abstract
Forty years ago, the Spanish training system for physicians, commonly called MIR, was created and now is a good time to reflect on its strengths and weaknesses. In 2014, a legal document named Royal Decree (RD), regulated, among other things, the core curricula specialties (named ´´troncalidad´´). For the almost two years that the RD 639/2014 was in force, numerous efforts were made by many physicians to improve our Specialized Healthcare Training system through residency. Since its cancellation in 2016, many challenges have emerged that will require the commitment of all of us to implement a new training system that really contributes to strengthen our health system.
