The combination of heart failure and ischaemic heart disease is common and associated with poor prognosis. In managing these patients, it is important to differentiate between myocardial damage, acute myocardial infarction type 1 and type 2, for which it is essential to take a clinical history and perform an electrocardiogram and troponin analysis. Immediate coronary angiography and revascularisation are indicated in the presence of heart failure or cardiogenic shock triggered by acute coronary syndrome. The medical treatment of acute coronary syndrome is no different to that of patients without heart failure. Patients with heart failure and/or left ventricular ejection fraction equal to or less than 40% will benefit from angiotensin-converting enzyme inhibitors, beta-adrenergic blockers, and mineralocorticoid antagonists.
