We present the case of a 35-year-old female patient who attended the emergency department of the Hospital Universitario Príncipe de Asturias (HUPA) due to continuous, stabbing pain in the right iliac fossa for 48 hours, which did not subside with analgesiaat home. She reported an increase in vaginal discharge of watery consistency, bloody appearance and dyspareunia for 3 months. She currently refers to a stable partner. The patient has had a levonorgestrel intrauterine device (IUD) since March 2018 with the following gestational formula (G1A0P1). Physical examination revealed a painful cervix on mobilisation. Uterus in anteroversion and regular. The right adnexa was painful on palpation. On the other hand, the transvaginal ultrasound showed a 5-centimetre right adnexal image compatible with a pyosalpinx. The patient was diagnosed with Pelvic Inflammatory Disease grade III (Monif Scale) and was admitted on suspicion of tubo-ovarian abscess. During admission, a culture of vaginal and endocervical exudate, haemogram, biochemistry and blood culture were requested and her evolution was monitored. Antibiotic therapy was prescribed for the patient and her partner, and the IUD was removed after the start of the treatment. The aim of this case is to highlight the importance of early diagnosis in the prevention of complications such as infertility, as well as to review the clinical management algorithm proposed by HUPA.
